All full-time faculty members may join the plan at the time of their employment. If a working spouse also qualifies for a medical policy at his/her employment, SCU will not be obligated to provide a second policy. Participation is voluntary. The “open enrollment” period is within 30 days of employment. After this initial period, evidence of insurability is required. The University pays the 80% of the premium and the faculty member, 20%. Upon retirement or termination of employment from the University, the insured may convert to an individual plan.
A membership application is available at the time of employment. Coverage is effective upon approval of the application for membership. When approved, a certificate of membership and a summary plan description which describes the benefits will be supplied to the employee.
